A new game from Belgian indie studio Clever Trickster Productions challenging a new genre Information and videos for ‘Sefton Asylum’ were released on Steam, and plans for a summer 2026 release were announced.
Set in an isolated mental hospital in the mid-20th century, this is a first-person horror game that forces players to make constant choices about whether to save patients or pursue the truth. Decisions made within limited time and resources lead to irreversible consequences, characterized by the delivery of pressure that drives the player into increasingly threatening situations.
The lone night nurse… Choices create horror
In ‘Sefton Asylum,’ the player takes on the role of a nurse working the night shift alone at a remote mental hospital. On the surface, she appears to be on duty to care for patients, but she has a hidden purpose. She volunteered for the night shift to track down her missing brother. However, the secrets hidden by Sefton Hospital were far darker than she had anticipated.
The core of the game lies in the pressure of repeated choices moment by moment. Players can spend time diagnosing and treating patients, or exploring forbidden zones to gather clues. However, untreated patients do not simply meet their end in death.
They transform the hospital into an increasingly threatening space, and eventually, simply surviving expands into another goal. The fact that they must endure an environment that becomes more dangerous without any weapons creates extreme tension.
‘Bio-real’ horror rooted in the Resident Evil series
Inspired by H.P. Lovecraft’s work *Herbert West-Reanimator*, the game focuses on reanimation, forbidden medicine, and bodily mutilation rather than vast cosmic horror. The overall atmosphere is closer to the “Bio-real” genre, represented by the Resident Evil series.
Based on graphics that realistically recreate the gloomy atmosphere of a mid-20th-century mental hospital, the game creates an extreme sense of claustrophobia and tension by utilizing worn lighting, rough textures, and limited visibility. The psychological pressure is also significant, combined with minimal sound design elements such as patients’ groans, distant metallic sounds, and sudden silences. It feels as though the focus was on realistic details and a gradually tightening atmosphere rather than exaggerated direction.
Lead Game Designer Egon explained, “We wanted players to personally experience the weight of the anxiety and guilt associated with their choices,” adding, “We gave strategic and emotional weight to every decision through a structure where abandoned patients return to pose a threat.” He also emphasized that the structural pressures and issues of undervaluation in the medical field were incorporated into the game’s narrative backdrop.
The five reunited after the studio’s bankruptcy, a product of reconstruction
‘Sefton Asylum’ holds more significance for the team than just a new release. Founded in 2023, Clever Trickster grew rapidly with 12 developers and released two titles, but due to changes in the external funding environment, the company eventually went bankrupt, leading to the dissolution of the entire team.
Although the entire team was laid off, five of them decided to make a fresh start based on a lighter structure and a clear sense of ownership. This work, ‘Sefton Asylum,’ is the result of that reconstruction that reflects their determination.
Producer Virginie stated, “Time pressure, limited resources, trade-offs, and cumulative results are the core of Sefton,” explaining that they applied the ‘system-centric design’ accumulated from the existing tycoon genre to a horror game.

Simultaneously with the release on April 3rd ‘Sefton Asylum’ has garnered attention from major gaming media outlets. COGconnected described it as “a tense experience that tightly fuses simulation, detective investigation, and psychological horror,” calling it “a highly anticipated title worth waiting for its summer release.”
All Hallows Geek also highlighted the studio’s rebuilding after bankruptcy, introducing it as “a horror game that faithfully captures the systemic DNA of Clever Trickster, with time pressure, resource limitations, and the consequences of choices at its core.”
Since the release of the video, reactions have been rapidly spreading across the Steam community and the horror game fandom. In particular, the game’s design, which uses real-world professional pressure as a source of horror, the paradoxical system where unsaved patients return as threats, and the gameplay mechanics that force players to choose between nursing and survival without weapons are drawing attention for their differentiation from existing horror games.
With the addition of the narrative of a five-person team rebuilt after overcoming the real adversity of bankruptcy, it is being evaluated as “a rare attempt to resolve the structural pressures of the medical system through game mechanics.”
